Medicaid : Service Coordination

As an 'Ohana member, you will be assigned to a service coordinator. The service coordinator is a trained health care professional. They will be your advocate within the plan, in the community and across the different health care services.

 

He or she will arrange, monitor and make sure you get timely care. The service coordinator will also make sure you get the right type of care. He or she will help you:

 

  • Arrange services and appointments
  • Get help with a provider
  • Coordinate care with your doctor(s) and other services
  • Develop a care plan with you that identifies your needs
  • Find answers to your questions about things like your benefits, medical care or medicines

The service coordinator will first get in touch with you by phone to find out about your:

 

  • Health and functional history
  • Existing services
  • Cultural/linguistic needs and preferences
  • Set up a face-to-face visit

The purpose of the face-to-face visit is to learn more about your health and functional history. This helps us to pair you with a service coordinator who best meets your needs. This visit will take place each year so we can update any changes you may have had. We will stay in touch with you regularly about your services and to update your plan of care.

 

'Ohana will also have nurses who can help by phone with urgent issues. These nurses will be able to help you 24 hours a day and 7 days a week, including holidays. The nurse may be able to answer many of your questions and help you when you are not feeling well. The nurse will also let your service coordinator and doctor know that you called and why.
