Medicare : Our Plans
'Ohana offers Medicare Advantage Plans that have more coverage than Original Medicare but without the cost of a Medicare Supplement Plan. Our Plans have an extensive network of quality health care providers and some of our plans have prescription drug coverage.
There are four easy ways to enroll:
Enrolling in one of our Medicare Advantage Plans is easy. Choose what works best for you.
- Enroll online - View our plans and complete your application online (click on the button above).

Enroll by mail - download, print and fill out our Enrollment Form
and mail it to 'Ohana by using the address within the form; or - Enroll over the phone - our representatives can enroll you right over the phone. Contact us to enroll; or
- Medicare beneficiaries may enroll in 'Ohana through the CMS Medicare Online Enrollment Center located at http://www.medicare.gov/.
If you want to enroll in a Special Needs Plan (Access, Select, Liberty, and Advocate Complete), please contact us for more information.

Members may enroll in the plan only during specific times of the year, unless you qualify for a special needs plan. Contact ‘Ohana for more information.
You will receive a notice via mail or e-mail acknowledging receipt of the completed enrollment application.
