Medication Therapy Management : Services

Comprehensive Medication Review

 

A Comprehensive Medication Review (CMR) is a consultation with a pharmacist or other qualified healthcare provider which assesses:

 

  • Why each medication is prescribed and whether any medications are potentially unnecessary
  • If each medication is effective
  • If each medication could be causing side effects, worsening other health conditions, or interacting with other medications
  • If a medication regimen can be made more affordable or simplified (such as fewer pills or a more convenient dosing schedule)
  • If medications are being taken appropriately to get the most benefit from them

 

The CMR is conducted over the phone with the member, and generally takes approximately 30 minutes. MTM members are not required to participate in a CMR, and may still remain in the MTM program if they choose not to participate in this service.

 

Launched in March 2010, the medication therapy management (MTM) call center at the University of Florida College of Pharmacy (UF COP) connects with 'Ohana Medicare Part D members throughout the United States who meet eligibility requirements of MTM services. Thousands of 'Ohana members benefit from this call center faculty, staff pharmacists and student pharmacists providing MTM services.

 

Educational Materials

 

Members enrolled in the Healthy Living Program will receive educational information on various health topics throughout the year. Topics covered may include nutrition and its affects on chronic conditions, the importance of taking medications correctly, coping with depression, and more!

 

Health and Medication Resource Center

 

Members also have complete access to an online health and medication resource center. Our website is a one-stop source of information on topics ranging from illnesses, first aid and treatment, prescription drugs, and medical tests and procedures.
