We are committed to transforming the health of the communities we serve, one person at a time. We partner with the State to provide managed care services to families—from keiki to kupuna—and individuals with the most vulnerable and complex medical needs across the islands.

Behavioral Health 

Community Care Services

Since 2013, ‘Ohana Health Plan has served as the sole provider of the state’s Community Care Services (CCS) program, a highly specialized care model to help individuals who have been diagnosed with serious mental illness (SMI) or serious and persistent mental illness (SPMI). 

These individuals are some of Hawaii’s most vulnerable people who have been diagnosed with:

  • Schizophrenia
  • Delusions
  • Psychosis
  • Bi-polar disorder
  • Major depression

For some, the impairment is such that they are unable to work or complete daily tasks independently. They are 3.5 times more likely to be homeless, often lack family or social support, and have high rates of mortality from preventable conditions like obesity and substance abuse.

SDOH: Addressing Housing Through Healthcare 

Community Integrated Services

From 2022 through June 2025, the ‘Ohana Community Care Services (CCS) Community Integration Services (CIS) program is proud to have housed a total of 475 members with SMI/ SPMI.

Stable housing is a social determinant of health and fosters positive stability, consistent treatment regimens, and positive health outcomes and improved quality of life for our members. This is achieved through close coordination of our housing management specialist collaborating with CIS providers and CCS case managers, along with existing housing programs and systems, such as Coordinated Entry System and Partners in Care’s Continuum of Care.

Total housed through CIS since 2022:

  • 326 CCS members
  • 171 QUEST (Medicaid) members

Innovative Solutions

CCS Residential Program for Integrated Care

In September 2024 ‘Ohana Health Plan and Hope Services Hawaii, Inc. launched a brand-new residential program for integrated care. This first-of-its-kind health plan and housing agency partnership in Hawaii will offer transitional housing and structured support for the most vulnerable and houseless ‘Ohana members. This program addresses a critical gap in housing support for our highest acuity members with physical and behavioral health needs that are too complex for traditional subsidized housing.

The eight-bed, Ainaloa-based home is dedicated exclusively for ‘Ohana Health Plan and aims to provide integrated, sustained clinical care and case management services. The treatment-based residential program provides:

  • BH case management
  • Licensed BH clinician for therapy and psychosocial rehabilitation groups
  • APRN for medication management
  • Housing case managers
  • Multi-disciplinary team working with health coordination for comprehensive care

This concentrated access to care should significantly reduce acute events for members, reducing acute emergency room or inpatient care expenses, and improving overall behavioral and physical health outcomes.

 

 

Maternal and Child Health

We know that a healthy pregnancy leads to healthy outcomes for both mom and baby. Mana ‘Ohana, our all-inclusive maternal and child health program, offers ‘Ohana Health Plan members exclusive access to a broad-based program designed to screen, triage, and support women and their newborns throughout the critical perinatal period. In partnership with Healthy Mothers Healthy Babies Coalition of Hawaii, the program provides culturally anchored and responsive outreach and services that meet members where they are.

In February 2025, ‘Ohana Health Plan and the Centene Foundation awarded a $500,000 grant to Hawaii Island Community Health Center to support their Gateway to Health project. The grant provides a new mobile health clinic to expand school-based health services for children in rural Hawaii Island. By driving out to these communities, the mobile health clinic will bring medical, dental, and mental health services directly to students. This approach helps meet the state health requirements, fostering a healthier school environment, reducing absenteeism, and enhancing academic performance.

Workforce Development

To address Hawaii’s nursing shortage, we partnered with The Caregiver Foundation to offer Certified Nurse Aide (CNA) scholarships and the University of Hawaii Foundation for the ‘Ohana Health Plan Pre-Nursing Scholarship at the University of Hawaii West Oahu. 

The CNA scholarships support training at the Advanced Care Training School in Wahiawa. Upon completion, graduates will receive licensing from the State of Hawaii and will dedicate their skills to serving the senior community of Hawaii.

The purpose of the ʻOhana Health Plan Pre-Nursing Scholarship is to assist students enrolled in the pre-nursing program at UH West Oahu, with funds to be used for costs associated with attendance (e.g. tuition, books, fees, etc.) and aims to develop and prepare scholarly leaders from West Oahu who are interested in improving the health and wellness of their community through a career in nursing.
