$10 – Update Member Information (Update your address, phone number, and email. Once per year, per person. $50 max per household).
Earn My Health Pays® rewards when you complete healthy activities by filling out the self-attestation form.
$40 – Annual Wellness Visit (All ages. Once per year).
$60 – Infant Well Child Visit (6) (Ages 0-14 months. Must have all six required visits completed before 15 months of life).
$20 – Infant Well Child Visit (2) (Ages 15-30 months. Must have the two required visits completed between 15-30 months of life).

Whole Family Well-Visit
Annual Wellness Visit – What better way to keep up to date with your health than schedule an appointment with your doctor primarily focused on preventative care, health screenings, and wellness planning. During an Annual Wellness Visit, you can expect to:
- Review your medical and family history
- Identify potential health risks
- Update list of prescriptions
- Get Immunization and receive screenings
- Get blood tests
- Get counseling and develop a personalized prevention plan
Infant Well Child Visit (6) and Infant Well Child Visit (2) – Well-child visits between the ages of 0-14 months and 15-30 months are crucial for a child’s growth and development. Key reasons well-child visits in this age range are important:
- Doctors monitor and track developmental milestones like language, motor skills, social interaction, and identify potential delays that might require early intervention
- Vaccinations to protect against preventable diseases
- Growth monitoring
- Address parental concerns
For My Health Pays®, you must take your child to six infant well-child visits on or before reaching 15 months old for the $60 reward and two infant well-child visits between 15 to 30 months for the $20 reward.
Pregnancy Program – Prenatal & Postpartum
Notification of Pregnancy (NOP) – Notification of pregnancy can help us enhance your plan of care to offer support services to ensure a healthy pregnancy and baby. Key benefits of an NOP:
- Early risk assessment with healthcare provider
- Personalized care plan
- Access to support services
- Care coordination
Healthy Mothers Healthy Babies Program – Start your pregnancy journey from prenatal to postpartum support with an assessment call with one of our representatives.
Postpartum Depression Screening – Your body and mind go through many changes in pregnancy. The combination of physical and emotional factors can lead to postpartum depression after having a baby. Complete a postpartum depression screening with your healthcare provider and get the support and counseling you need.

You will receive your My Health Pays Visa® Prepaid Card when you attest to completing your first healthy activity from ‘Ohana Health Plan. Each time you attest to completing a qualifying healthy activity, we are notified, and your reward dollars will be added to your existing card. This card is issued by The Bancorp Bank, N.A., Member FDIC, pursuant to a license from Visa U.S.A. Inc. Card cannot be used everywhere Visa debit cards are accepted. Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact us toll free at 1-888-846-4262 (TTY 711) Monday-Friday, 7:45 a.m. to 4:30 p.m. HST.
Once you receive your My Health Pays rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation.

Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
